I am an in-network provider with Excellus Blue Cross Blue Shield and MVP Health Care. If you plan to use your insurance please be sure to call ahead and verify what your benefits are related to nutrition services. If I do not accept your insurance you still have the option to submit your receipt with my information to your insurance company for potential reimbursement. All services and copays must be paid by cash, card, or check at the time of your appointment.
My "Health Insurance Verification Form" will guide you through the questions you need to ask your insurance company to determine what type of coverage your specific plan has for your diagnosis or needs related to nutrition services.

*A credit or debit card is required at the time of booking in order to secure your time slot.
*All clients are required to verify their own health insurance benefits by completing the "Health Insurance Verification Questionnaire" digital form prior to your first appointment. Any payments that are denied by your insurance company will be charged to the credit or debit card on file.
*A $50 cancelation fee is applied to the credit or debit card on file if no notification is provided prior to 24 hours before your first scheduled appointment and any follow up appointments.
*All forms must be completed digitally at least 24 hours before your first appointment.
*Masks are required in the waiting areas of the building.
